Housing Stabilization Services (HSS) is divided into three key services that work together to support individuals in securing and maintaining stable housing. The first service, Housing Consultation, helps individuals who do not have a case manager develop a person-centered housing plan and select a housing provider. The second service, Housing Transition, provides support for finding and moving into a new home in the community. Finally, Housing Sustaining focuses on helping individuals keep their housing through ongoing support, education, and coordination with landlords and community resources. Together, these services form a comprehensive approach to long-term housing stability.
People who do not have waiver or targeted case management or a MSHO/MSC+ care coordinator, but need Housing Stabilization Services, can complete the required person-centered planning through an enrolled housing consultant.
Housing consultants help develop a Housing Focused Person-Centered Plan (DHS-7307) and support individuals in selecting their housing transition or sustaining provider.
