Housing Stabilization Services

Housing Stabilization Services (HSS) is divided into three key services that work together to support individuals in securing and maintaining stable housing. The first service, Housing Consultation, helps individuals who do not have a case manager develop a person-centered housing plan and select a housing provider. The second service, Housing Transition, provides support for finding and moving into a new home in the community. Finally, Housing Sustaining focuses on helping individuals keep their housing through ongoing support, education, and coordination with landlords and community resources. Together, these services form a comprehensive approach to long-term housing stability.

Step 1

Housing Consultation

 

People who do not have waiver or targeted case management or a MSHO/MSC+ care coordinator, but need Housing Stabilization Services, can complete the required person-centered planning through an enrolled housing consultant.


Housing consultants help develop a Housing Focused Person-Centered Plan (DHS-7307) and support individuals in selecting their housing transition or sustaining provider.

Covered Services (Housing Consultation)

  • Assist with accessing documentation required for Housing Stabilization Services eligibility
  • Develop a Housing Focused Person-Centered Plan based on assessment outcomes 
  • Support the person in identifying housing strengths, needs, wants (including cultural requirements/preferences)
  • Help the person make an informed choice in selecting a housing transition or sustaining provider
  • Provide information for services supporting non-housing-related goals
  • Coordinate with other current service providers
  • Help the person understand rights to privacy and appeal processes
  • Annually update the person-centered plan as it relates to housing

Step 2

Housing Transition Services

 Services that help a person plan for, find, and move to a home in the community. 

Services NOT Covered

 

  • Room and board (including moving expenses)
  • Deposits
  • Food 
  • Furnishings 
  • Rent 
  • Utilities
  • No-show appointments 
  • Staff travel time (when the person is not in the vehicle or housing isn’t discussed)
  • Group setting services
  • Transition services if the person dies before discharge OR transition services exceed 180 days
  • Physically touring housing located outside of Minnesota

Step 3

Housing Sustaining Services

 Services that help a person maintain living in their home in the community. 

Services Covered

 

  • Develop, update, and modify the housing support and crisis/safety plan regularly*
  • Identify and prevent behaviors that may threaten housing stability
  • Educate/train on tenant and property manager roles, rights, responsibilities 
  • Provide transportation with the person present, discussing housing issues
  • Support/promote cultural practices with landlords, property managers, neighbors*
  • Coach to maintain relationships with neighbors/property managers 
  • Advocate with community resources to prevent eviction and ensure safety* 
  • Help with housing recertification processes*
  • Train on lease compliance, household management, being a good tenant
  • Support applications for benefits to retain housing*
  • Support understanding and increasing income/benefits to retain housing*
  • Build natural housing supports and culturally aligned community resources
  • Work with landlords to promote housing retention*
  • Arrange for assistive technology* 
  • Arrange for adaptive housing accommodations*
